Youth Transition Programs The Youth
Transition Program (YTP) focuses on providing youth with disabilities, ages
17-21, with assistance in transitioning from school to post secondary education
and training, employment and independent living. YTP participants must have some
type of documented disability and be eligible for services with the Office of
Vocational Rehabilitation Services (OVRS) to be eligible for YTP. Referrals to
the program are made in coordination with the student’s school, CSC and OVRS. The youth are
served by a transition team consisting of a Vocational Rehabilitation Counselor
from OVRS, a Transition Specialist provided by CSC, and a Special Education
Instructor from the local school district, who together with the participant
develop a transition plan. During this program youth are given a variety of
services which may include: career exploration, job shadows, unpaid and paid
community work experiences, mentoring, classroom instruction in academic and
vocational skills, independent living and personal/social skills and on-the-job
